Sompo Holdings will respond appropriately to requests such as those for notification of the purpose of use, disclosure, revision, and suspension of use of retained personal data under the Act on the Protection of Personal Information, etc. (hereinafter referred to as "Request for Disclosure").
1. How to make a request
If you wish to make Request for Disclosure, please contact the contact point below. We will send you our designated written form, and ask you to fill in the necessary information and submit it together with the following documents at the designated contact point.
(1)If the person making the request is the principal:
A copy of a document issued by a public institution, such as your driver's licence, passport, health insurance card or pension book.
(2)If the person making the request is an agent:
In addition to the documents confirming the identity of the agent (same as (1) above), please submit the following documents.
In the case of a legal representative, a copy of the family register, a copy of the adult guardianship registration certificate or other document that confirms the legal representation.
In the case of a voluntary representative, his/her letter of attorney and certificate of seal registration.
2. Fees
For "notification of the purpose of use" and "disclosure" of retained personal data, we will charge 700 yen (including tax) as a fee, please transfer the amount to the designated bank account of our company.
Please note that the customer will be responsible for the mailing costs when he/she sends us Request for Disclosure and the bank transfer fees when they transfer the fees to us. Please understand this in advance.
3. Method of Response
For Request for Disclosure that we receive, we will confirm and investigate the contents, and for requests that require a fee, we will confirm the payment and reply to the person in question, in principle, by the method requested by him/her among the methods specified by us. In the case of a request from a representative, we will reply to that representative.
Please note that we may not be able to comply with your request if there is a risk of harm to human lives bodies, property or other rights or interests, if there is a risk of significant obstruction to the proper conduct of our business, or if it would be in breach of other laws or regulations, etc. In such cases, we will inform you of the reason.
